第二代基因杂交捕获法检测高危型HPV-DNA在宫颈癌前病变筛查中的应用研究

摘    要:目的探讨第二代基因杂交捕获(HC2)法检测人乳头瘤病毒(HPV)分型基因对宫颈癌前病变的诊断作用及意义。方法选择北京市顺义区妇幼保健院2009年1月~2012年1月不同年龄妇女宫颈脱落细胞标本2400例,采用HC2法检测高危型HPV-DNA,并与液基薄层细胞检测(TCT)进行对比,异常患者行病理组织学检查,以病理结果为金标准,比较HC2法与TCT的诊断效果。结果罹患宫颈癌及癌前病变的女性以青中年为主。TCT检查、HPV检测与病理的符合率是随病理级别的上升而上升。CINⅢ及宫颈癌与高危型HPV感染密切相关,其中以HPV16、18、58为主。结论高危HPV感染是宫颈癌及癌前病变发病的主要致病因子,HPV16型感染易导致宫颈高度病变;HPV-DNA联合TCT能最大程度地发现宫颈异常细胞并及时发现宫颈癌的诱因。

Abstract:Objective To investigate the HC2 hybridization assay for detection of human papilloma virus(HPV) geno typing in cervical precancerous lesions diagnosis and significance.Methods 2400 cases of women in different age in exfoliative cervical cell specimens in Maternal and Child Health Hospital of Shunyi District from January 2009 to Jan uary 2012 were selected;high risk HPV-DNA was detected by HC2 method,TCT was compared with HC2;histopathological examination was used to detect the abnormal patients,pathological result was the golden standard.Effect of diagnosis between the HC2 method and TCT were compared.Results The majority of cervical cancer and precancerous lesions were the young and middle-aged women.TCT,HPV detection and pathology of the coincidence rate were found ascent with pathological levels rise.CIN Ⅲ,cervical cancer were found related with high-risk HPV infection,HPV16,18,58 were the majority.Conclusion The high risk of HPV infection is the major pathogenic factors of cervical cancer and precancerous lesions,type HPV16 infection in cervical lesions can lead to height of cervical lesions;HPV-DNA combined with TCT can find the abnormal cervical cells and discover the cause of cervical cancer.
