摘    要:目的评价应用动力加压接骨扳(DCP)和锁定加压接骨板(LCP)固定联合松质骨移植治疗肱骨干骨折术后无菌性骨不连的效果,并进行临床对比分析。方法回顾性分析2000年1月~2010年2月在我院骨科行手术治疗的肱骨干骨折术后无菌性骨不连的患者37例、采用平均手术时间、临床痊愈时间、功能恢复以及并发症来评估两种接骨板的临床获益。结果两组年龄和性别比无统计学差异(P〉0.05);平均手术时间DCP组(1492±11.8)min,LCP组(1428±127)min(P〉0.05);愈合时间DCP组(136±142)d,LCP组(119±108)d,有统计学差异(P〈0.05);并发症发生率两组无统计学差异(P〉0.05),关节功能评价优良率LCP组优于DCP组(P〈0.05)。结论加压接骨板联合松质骨移植是治疗肱骨干骨折术后无菌性骨不连的有效方法;LCP较DCP能缩短病程、更好地恢复伤肢功能。

Abstract:Objective We compared the effect of dynamic compression plate (DCP)with cancellous bone graft and locking compression plate (LCP) with can cellous bone graft for aseptic nonunion after surgical treatments of humeral shaft fracture. Method thirty-seven patients with aseptic nonunion of humeral shaft fracture after various surgical treatments were reviewed and analyzed retrospective]y between January 2000 and February 2010.With the average operation time, the range of motion of shoulder and elbow, the healing time and complications to assess the clinical benefit. Result There was no significant difference between age and gender in the DCP group and LCP group (P〉0.05). The average operation time was (149.2±11.8) min (DCP group), and (142.8±12.7) rain (LCP group), showing no significant difference (P〉0.05). The healing time was (136±14.2) d in DCP group, and (119±10.8) d in LCP group respectively, showing significant difference (P〈0.05). The shoulder and elbow function of LCP group is superior to DCP group (P〈0.05). No significant difference in complications of the two groups (P〉0.05). Conclusion the compression plate with supplemental cancellous bone graft is an effective and reliable treatment for aseptic nonunion of humeral shaft fracture after surgical treatment; fixation by LCP can reduce the union time and better recovery.
