认知损害老年人睡眠呼吸障碍的家庭睡眠监测

目的通过家庭睡眠监测,结合睡眠及神经心理量表,研究伴有认知损害的阻塞性睡眠呼吸暂停综合征(OSAS)老年人睡眠及认知功能情况,分析认知损害与OSAS的相关性,探讨利用家庭睡眠监测研究老年OSAS的可行性。方法49例受试者(OSAS18例,对照31例)检查匹兹堡睡眠量表(PSQI)及爱泼沃斯睡眠量表(ESS)及多导睡眠图(PSG)家庭监测,及简短智力状态检查(MMSE)、画钟测验、词表学习、符号数字测验等认知功能检查。结果(1)OSAS老年人Delta睡眠比例、最低血氧水平明显减少、氧减指数明显增加(P<0.05);(2)OSAS老年人记忆减退持续时间较长、数字符号测验成绩较差(P<0.05);(3)数字符号测验成绩与PSQI结果显著负相关(r=-0.3026,P<0.05)。结论(1)老年人认知损害与OSAS具有一定相关性;(2)家庭睡眠监测可以较好地研究伴有认知损害老年人的OSAS情况。

Sleep monitoring at home in the elderly with obstructive sleep apnea syndrome induced by cognitive impairment

Objective To study the relationship between obstructive sleep apnea syndrome (OSAS) and cognitive impairment in the elderly using neuropsycho logical test and polysomnography at home so as to explore the feasibility of OSA S study by sleep monitoring at home. Methods 18 patients with OSAS and 31 controls were tested by polysomnography at home, Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale(ESS), Mini-Mental State Examination(MMSE), cloc k drawing, word list recall and digital symbol test. Results The delta sleep ratio was significantly shortened and lowest saturation decreased significantly as well but oxygen desaturation index increased in the OSAS subjects (P<0.05). Performances on the digital symbol test in the OSAS were significantly impaired compared with the control group, which were significantly associated with PSQI ( r=-0.3026, P<0.05). Conclusion OSAS is correlated to cognitive impairment. Sleep monitoring at home is feasible in studying OSAS and cognitive impairment in the elderly patients.
