辣椒辣素对大鼠膀胱逼尿肌作用的实验研究

摘    要:目的 探讨辣椒辣素 (capsaicin ,CAP)对大鼠膀胱功能的影响及其作用机制。 方法 将 2 5只Wistar大鼠分为 5组 ,分别用 0 .9mmol/L、1.8mmol/L、3.6mmol/L、7.2mmol/LCAP作用于大鼠膀胱 ,用生理盐水作对照。观察用药前后膀胱内压力变化、膀胱容量及残余尿量。另取大鼠膀胱三角区组织用PAP法进行免疫组化染色 ,观察P物质 (SP) ,根据免疫组化SP阳性神经纤维的密度和染色强度分为 4级 ,即可疑 (± )、轻度阳性 (+) 2~ 4个阳性纤维 ,染色差 ;中度阳性 (++) 5~ 6阳性纤维 ,染色较强 ;强阳性 (+++) 7~ 9阳性纤维 ,染色很强。结果 ①低浓度组 0 .9mmol/LCAP组和 1.8mmol/LCAP组与对照组比较 ,其膀胱内压、膀胱容量和残余尿量均无显著性差异 ;SP分布也无明显差异 (P >0 .0 5 )。②高浓度组 (3.6mmol/LCAP组和 7.2mmol/LCAP组 )膀胱内压明显降低 ,残余尿量增加 (P <0 .0 1) ;③高浓度组膀胱组织内SP阳性神经纤维明显减少 ,染色明显减弱 (P<0 .0 1)。结论 高浓度辣椒辣素可抑制膀胱逼尿肌收缩 ,抑制排尿 ;另外高浓度CAP能耗竭膀胱内感觉神经末梢肽类介质SP。说明CAP通过消耗膀胱内的SP抑制感觉信号的传递 ,使逼尿肌不能正常收缩而抑制排尿

Abstract:Objective To investigate the effect of capsaicin (CAP)on the bladder function of murine.Methods An in vivo study was performed on the bladder of 25 Wistar rats. The rats were divided into one control group and four experimental groups. Cystometry was performed. Catheterised bladders were connected to a multifunctional manometer. Increasing concentrations of CAP( 0.9 ?mM, 1.8 ?mM, 3.6 ?mM and 7.2 mM)were instilled into the bladder separately. Intravesical pressure, detrusor contraction and micturition status were recorded. Substance P(SP) was measured in the trigone of the bladder by immunohistochemical assay.Results The maximal detrusor pressure and capacity of bladder were slightly increased at low concentrations of CAP( 0.9 ?mM, 1.8 ?mM),( P > 0.05 )but were significantly decreased at high concentrations of CAP( 3.6 ?mM and 7.2 ?mM),( P < 0.01 ). The depletion of SP is more significant in the groups treated with high concentrations of CAP( 3.6 ?mM and 7.2 ?mM),( P < 0.01 ).Conclusions CAP depletes SP in sensory afferent nerve terminals and inhibits the detrusor contractility. The result implies that local application of CAP in the bladder could be used in the treatment of neurogenic bladder(detrusor hyperreflexia).
