自体全血注射联合抗组胺药治疗慢性荨麻疹血清皮肤试验阳性患者的效果评价

目的探讨自体全血注射联合抗组胺药物治疗慢性荨麻疹血清皮肤试验（ASST）阳性患者的临床疗效及其对炎症因子水平l拘影响。方法将82例ASST阳性的慢性荨麻疹患者随机分为观察组和对照组，每组41例。2组患者均给予氯雷他定片（5mg／次，1次／d）口服治疗，治疗2周后根据患者疗效将剂量改为隔日1次，对比分析2组患者治疗效果。结果治疗后观察组皮肤瘙痒程度、风团大小、UAS评分改善效果优于对照组（P〈005）；观察组治疗后血浆组胺含量及IL-17、IL-18、IL-23下降水平显著低于对照组（P〈0．05）；观察组总有效率为9268％，显著高于对照组的78．05％（P〈0．05）。结论自体全血注射联合氯雷他定片治疗ASST阳性荨麻疹安全有效，可明显改善患者临床症状及炎症因子水平，有利于患者预后及恢复。

Effect of autologous blood injection combined with antihistamines in the treatment of chronic urticaria patients with positive serum skin test

Objective To investigate the clinical efficacy of autologous blood？ injection combined with？ antihistamines in the treatment of chronic urticaria patients with positive serum skin test （ASST） and its effect on the level of inflammatory cytokine. Methods Eighty-two ASST positive chronic urticaria patients were randomly divided into observation group （n = 41）and control group （n = 41）. Two groups were treated with loratadine tablets （5 mg/time, 1 time/d, oral in- take）. After 2 weeks of treatment, the dose was reduced to once every other day based on the effi- cacy. The therapeutic effects of the two groups were compared and analyzed. Results After treat- ment, the improvement of skin pruritus, wheal size, and UAS score was better in the observation group than in the control group （P〈0.05）. After treatment, plasma histamine and IL-17, IL-18, IL-23 levels had more decreases in the observation group than in the control group （P 〈 0.05）. The total effectiveness rate was 92.68 % in the control group, notably higher than the 78.05 % in the control group （P 〈 0.05）. Conclusion Autologous blood injection combined with loratadine tablets in the treatment of ASST positive urticaria is safe and effective, it can improve clinical symptoms and inflammatory cytokine levels, and it is conducive to the prognosis and recovery of patients.
