2型糖尿病周围神经病变患者的血脂分布情况及相关危险因素分析

目的探讨高密度脂蛋白对2型糖尿病周围神经病变患者的影响。方法选择2008—2012年本院收治的80例2型糖尿病伴神经周围病变患者作为研究组，另外随机选取80例无周围神经病变的2型糖尿病作为对照组，对比分析2组患者的一般资料及血糖血脂指标。结果研究组有糖尿病家族史者多于对照组（P〈0．05），病程明显较对照组长（P〈0．05）。2组Hb舢C、FBG、TC水平差异无统计学意义（P〉0．05）；研究组HDL、HDL2、HDL3均低于对照组，LDL、LDLa、TG高于对照组，差异有统计学意义（P〈0．05）。多因素Logistic回归分析结果显示：病程（B=2．417，OR=9．996）和HDL2（p=-1．336，OR=6．752）是糖尿病患者周围神经病变的独立危险因素（P〈0．05）。结论糖尿病患者发生周围神经病变可能与病程及HDL2水平下降有关，对于以上类型患者应进行综合性调脂治疗，从而防治周围神经病变。

Lipid distribution and analysis of related risk factors in patients with type 2 diabetic peripheral neuropathy

Objective To explore the influence of high - density lipoprotein on treatment of patients with type 2 diabetic peripheral neuropathy. Methods 80 patients with type 2 diabetic pe-ripheral neuropathy from 2008 to 2012 were collected and designed as research group, while 80 type 2 diabetes patients without peripheral neuropathy were collected and designed as control group. General materials and levels of blood glucose and lipid were compared and analyzed between two groups. Results Number of patients with family history of diabetes in the research group was larg- er than that in the control group （P 〈 0.05）, and course of disease was significantly longer than that in the control group （P〈 0.05）. There were no significant differences of HbAlc, FBG and TC levels between the two groups （P 〉 0.05）. The levels of HDL, HDL2 and HDL3 in the research group were lower than those in the control group, and the levels of LDL, LDLa and TG were higher than those in the control group （P 〈 O. 05）. Multivariate Logistic regression analysis showed that the course of disease （β = 2. 417, OR = 9. 996） and HDL2 （β = - 1. 336, OR = 6. 752） were the in- dependent risk factors for diabetes patients with peripheral neuropathy （ P 〈 0.05）. Conclusion Diabetic patients with peripheral neuropathy may be associated with course of disease and decline of HDL2 level. Comprehensive lipid regulation therapy is useful for prevention and treatment of pe- ripheral neuropathy.
