适形放疗联合替吉奥治疗老年食管癌的疗效分析

目的探讨适形放疗联合替吉奥治疗老年食管癌患者的近期疗效及安全性。方法将40例老年食管癌将患者随机分为放疗联合同步替吉奥组（替吉奥组）和放疗联合同步DF方案组（DF组），每组20例，替吉奥组采用三维适形放疗联合替吉奥进行治疗，DF组采用三维适形放疗联合顺铂＋氟尿嘧啶＋左亚叶酸钙进行治疗，比较2组患者的治疗完成情况、近期疗效及不良反应发生情况，并按照东部肿瘤协作组（E∞G）标准评价患者。结果替吉奥组完成同步放化疗率显著高于DF组，Ⅲ～Ⅳ级消化道反应及白细胞减少发生率均显著低于DF组，且ECOG评分改善显著优于DF组，差异均有统计学意义。而2组近期疗效比较，差异无统计学意义。结论适形放疗联合替吉奥能有效改善老年食管癌患者的日常体能状态，提高生活质量，且不良反应轻，患者依从性好。

Analysis on therapeutic effect of conformal radiotherapy combined with S- 1 for elderly patients with esophageal carcinoma

Objective To explore the clinical effect and safety of eonformal radiotherapy combined with S- 1 for elderly patients with esophageal carcinoma. Methods Forty elderly patients with esophageal carcinoma were randomly divided into radiotherapy plus concurrent S- 1 group （S - 1 group） and radiotherapy plus concurrent DF regimen group （DF group）. S- 1 group was treat- ed with radiotherapy and S - 1, while DF group with radiotherapy plus cisplatin ＋ fluorouracil ＋ cal- cium levofolinate. The conditions of finishing treatment, clinical effects and adverse reactions were compared in two groups, and daily performance status of Eastern Cooperative Oneology Group （ECOG） was evaluated. Results The rate of completing concurrent chemoradiotherapy in S- 1 group was markedly higher than in DF group, incidences of digestive reactions at grades ]]1 - IV and leucopenia significantly lower than in DF group and ECOG score obviously higher than in DF group, with significant differences. There was no significant difference compared with short- term effects of two groups. Conchlsion Conformal radiotherapy combined with S- 1 can effectively ameliorate daily performance status of elderly patients with esophageal carcinoma, improve the quality of life, has mild adverse reactions and good compliance.
