胰十二指肠切除术和保留十二指肠胰头切除术治疗慢性胰腺炎的临床疗效分析

目的:探讨胰十二指肠切除术和保留十二指肠胰头切除术治疗慢性胰腺炎的临床疗效。方法:回顾性分析60例慢性胰腺炎患者的临床资料。结果:胰十二指肠切除术组30例患者,经过常规的Whipple术式治疗,发生并发症8例,其中胆肠吻合口狭窄1例,胰漏4例,胆漏3例,发生率为26.7%,住院时间为(14.9±5.3)d;保留十二指肠的胰头切除术组30例患者,胰腺空肠吻合口狭窄2例,并发症发生率为6.7%,住院时间为(8.4±3.0)d;两组的并发症发生情况、住院时间经统计学比较分析,差异有统计学意义(P<0.05);疼痛缓解情况比较,差异无统计学意义,P>0.05。结论:保留十二指肠的胰头切除术治疗慢性胰腺炎的临床疗效更为确切,值得临床推广使用。

The clinical efficacy comparison between pancreatoduodenectomy and duodenum-preserving pancreatic head resection for chronic pancreatitis

Objective To investigate the difference of the pancreatoduodenectomy and duodenum-preserving pancreatic head resection for chronic pancreatitis in the clinical efficacy.Method A retrospective analysis was made on the clinical data of 60 cases with chronic pancreatitis admitted to hospital in recent years.Results Pancreaticoduodenectomy group of 30 patients after conventional Whipple procedure,and 8 cases of complications,including biliary-enteric anastomotic stricture in 1 case,4 cases of pancreatic leakage,biliary fistula in 3 cases,the rate was 26.7%,hospital stay was （14.9 ± 5.3） d;duodenum-preserving pancreatic head resection group of 30 patients,pancreatic anastomotic stenosis in 2 cases,complication rate was 6.7%,hospital stay （8.4 ± 3.0） d;two groups of complications,hospital stay compared by statistical analysis,there was significant difference （P0.05）;pain relief compared to the situation,no significant difference（P0.05）.Conclusion The clinical efficacy of duodenum-preserving pancreatic head resection in the treatment of chronic pancreatitis is more accurate,is worthy of clinical use.
