腓肠神经营养血管逆行岛状皮瓣修复内踝皮肤缺损的临床研究

摘    要:目的 探讨腓肠神经营养血管逆行岛状皮瓣修复术对内踝皮肤缺损的治疗效果,为以后的治疗提供更科学的指导与参考.方法 筛选从2004年2月至2011年3月期间在我院住院治疗的内踝皮肤缺损患者36例,将其分为观察组18例和对照组18例.观察组行腓肠神经营养血管逆行岛状皮瓣修复术,对照组患者行邻近皮瓣修复术,对比两组患者术后皮瓣成活情况.结果 观察组成活率明显高于对照组,坏死率显著低于对照组,差异均具有统计学意义(P <0.05);对照组平均下床所用时间、平均住院时间明显较观察组长,两组比较差异具有统计学意义(P<0.05).结论 对内踝皮肤缺损的患者实施腓肠神经营养血管逆行岛状皮瓣修复术,可以取得良好效果,术后出现坏死情况通过及时的对症处理能有效治愈,是内踝皮肤缺损患者理想的治疗方案.

Clinical Study of Repairing Skin Defect of Medial Malleolus by Sural Nerve Island Flap Pedicled with Collateral Vessels

Abstract:Objective To investigate the effects of repairing skin defect of medial malleolus by sural nerve island flap pedicled with collateral vessels, so as to provide guidance and reference for scientific treatment. Methods Thirty-six patients with skin defects of medial malleolus in our hospital from February 2004 to March 2011 were selected and divided into observation group and control group, with 18 cases in each group. The observation group received sural nerve island flap pedicled with collateral vessels, while the control group received adjacent flap repair. The effects of two groups were compared. Results The survival rate of observation group was obviously higher than that of control group, the necrosis rate was significantly lower than that of control group, the differences were statistically significant (P〈0.05); The control group had longer average out-of-bed time and average length of stay compared with the observation group (P 〈0.05). Conclusions The implementation of repairing skin defect of medial malleolus by sural nerve island flap pedicled with collateral vessels can get good effect, and postoperative necrosis can be effectively cured through timely symptomatic treatment. It is an ideal treatment method for patients with skin defect of medial malleolus.
