MR脂肪抑制FIESTA在鉴别诊断肝脏囊肿血管瘤中的临床意义

目的总结MRI脂肪抑制二维稳态采集快速成像（2DFIESTA）在肝脏血管瘤和囊肿鉴别诊断中的临床意义。方法分析MRI多期动态增强证实的65例肝脏血管瘤和178例肝囊肿，2D脂肪抑制FIESTA序列中的信号强度变化特征。结果2D脂肪抑制FIESTA序列图像肝血管瘤不论大小，病灶信号与肝内胆管或胆囊信号相比均有衰减，但仍呈高信号，肝囊肿最大直径小于等于3cm时，96．2％的病灶信号与肝内胆管或胆囊信号相似；大于3cm的病灶信号均与肝内胆管或胆囊信号相似或略高。结论2D脂肪抑制HESTA序列可以鉴别诊断绝大多数的肝脏血管瘤和囊肿，而无需静脉增强扫描。

The value of MR two-dimensional Fast Imaging Employing Steady-state Acquisition(2D FIESTA)in the distinguishing diagnosis of hepatic hemangioma and hepatic cyst

Objective To evaluate the value of MR two-dimensional stable state fast gathering image formation sequence（2D FIESTA）in the distinguishing diagnosis of hepatic hemangioma and hepatic cyst. Methods 65 hepatic hemangioma cases and178 hepatic cyst cases were examined with MRI and verified by dynamic enhanced MRI,and analyzed the significant change of signal strength in 2D FIESTA sequence. Results Singal of hepatic hemangioma in 2D FIESTA is defined faint with the gallbladder signal intensity. But still were higher than intermediate signal intensity, hepatic cyst（ ≤3 cm） ,96. 2% signals in focuses of infection and the liver is similar to signal of the bile duct or the gallbladder; signal（ 〉 3 cm） in the focus of infection is similar to or slightly higher than signal of the bile duct or the gallbladder. Conclusion 2D the FIESTA sequence may distingnish and diagnosis the majority of hepatic hemangioma and hepatic cyst without contrast-enhanced MR.
