急诊ERCP治疗急性胆源性胰腺炎54例临床分析

目的观察急诊内镜逆行胰胆管造影术(ERCP)治疗急性胆源性胰腺炎(ABP)的疗效。方法回顾性分析2007年6月至2011年6月我院消化内科行早期ERCP治疗的54例ABP患者的临床资料,并与同期开腹治疗的30例ABP患者进行比较。结果 54例患者均顺利完成ERCP,未发现与内镜操作有关的严重并发症。两组患者的腹痛缓解时间、肠道恢复通气时间、血清淀粉酶/胆红素/白细胞计数/体温恢复正常时间、住院时间等比较差异均有统计学意义(P<0.05)。ERCP组的疗效明显优于开腹组。结论 ERCP治疗ABP创伤小,疗效确切,患者恢复快,可作为治疗ABP的理想方法之一。

Clinical Analysis of 54 Cases of Emergency ERCP to Treat Acute Biliary Pancreatitis

Objective To observe the effect of emergency endoscopic retrograde cholangiopancreatography(ERCP)in treatment of acute biliary pancreatitis(ABP).Methods Clinical data of 54 patients with ABP from June 2007 to June 2011 in early ERCP treatment were retrospectively analyzed,and compared to 30 patients with ABP using open surgery during the same period.Results All of the 54 patients successfully completed ERCP,the related severe complications of endoscopic operation was not found.The abdominal pain relief time,intestinal recovery ventilation time,serum amylase/bilirubin/white blood cell count/body temperature to restore the normal time,hospitalization time had statistically significant differences(P<0.05).The curative effect group ERCP was obviously superior to the open group.Conclusion ERCP treating ABP had small wound,affirmative effect and fast recovery,which can be used as one of the best methods in treatment of ABP.
