双源CT对先天性心脏病心外畸形的诊断价值

目的:探讨双源CT(DSCT)在先天性心脏病心外畸形中的诊断价值。方法:回顾性分析、对比117例先天性心脏病心外畸形的术前DSCT和超声心动图(Echo)的影像资料,并与手术结果对照。结果:全部检查均符合诊断要求,发现心外结构异常304处,DSCT可清晰显示先心病心外畸形特点及侧枝循环情况,DSCT、Echo及二者联合对心外结构异常诊断准确率分别为96.7%(294/304)、78.9%(240/304)9、7.7%(297/304),DSCT对心外畸形的诊断准确率明显高于Echo,二者差异具有统计学意义(P0.001),而DSCT与二者联合对心外结构异常的诊断准确率差异无统计学意义(P0.05)。结论:DSCT是先天性心脏病的一项重要无创检查技术,各种重建图像能够实时、全面、直观地显示先天性心脏病各种心外结构异常,检出优于Echo,对先心病特别是复杂型先心病的手术方案制定有重要的临床意义。

The study of dual source computed tomography angiography in the evaluation of of extracardial anomalies in patients with congenital heart disease

Objective：To explore the value of dual-source CT（DSCT） angiography in diagnosis of extracardial anomalies in patients with congenital heart disease.Methods：Imaging data of DSCT and Echo obtained in 117 patients before operatively were analyzed retrospectively,and compared the results of surgery.Results：There were 304 abnormal stuctures,the diagnostic accuracy of CT and Echo,the total accuracy of CT and Echo for the extracardiac anomalies were 96.7%（294/304）,78.9%（240/304） and 97.7% respectively.There were statistically significant differences between DSCT and Echo.There was not statistically significant difference between DSCT and the total accuracy of DSCT and Echo.Conclusion：DSCT is an important noninvasive examination technique of congenital heart diseases,the reconstructed images of DSCT can descover the extracardiac anomalies of congenital heart diseases in real time and directly.DSCT is superior to Echo in the diagnosis of extracardiac anomalies,and have important clinical value to make operative plan for congential heart.
