高分辨磁共振对椎基底动脉夹层动脉瘤的临床诊断价值研究

目的 探讨高分辨磁共振对椎基底动脉夹层动脉瘤的临床诊断价值.方法 选取我院2012年3月至2014年3月期间收治的80例疑似椎基底动脉夹层动脉瘤患者为研究对象,对所有患者行高分辨磁共振检查（HRMRI）诊断和数字减影血管造影（DSA）诊断,比较两种诊断方式的符合率.结果 手术及病理证实80例疑似患者中43例为椎基底动脉夹层动脉瘤,高分辨磁共振诊断40例为椎基底动脉夹层动脉瘤,数字减影血管造影诊断28例为椎基底动脉夹层动脉瘤,高分辨磁共振诊断符合率（93.02％）显著高于数字减影血管造影诊断符合率（65.12％）,差异具有统计学意义（P＜0.05）.结论 高分辨磁共振可提供椎基底动脉夹层动脉瘤病变处的管腔信息和管壁信息,具有可重复性、无创性等优势,对椎基底动脉夹层动脉瘤的诊断具有重要的价值.

Clinical Diagnostic Value of High Resolution Magnetic Resonance Imaging on Vertebrobasilar Artery Dissecting Aneurysm

Objective To investigate the clinical diagnostic value of high resolution magnetic resonance imaging on vertebrobasilar artery dissecting aneurysm. Methods 80 cases of patients with suspected vertebrobasilar artery dissecting aneurysms treated in our hospital from March 2012 to March 2014 were selected. All the patients received high resolution magnetic resonance imaging （HRMRI） and digital subtraction angiography （DSA）. The coincidence rate of two diagnostic methods were compared. Results 43 cases ofvertebrobasilar artery dissecting aneurysm were confirmed by operation and pathology, 40 cases of vertebrobasilar artery dissecting aneurysm were confirmed by high resolution magnetic resonance imaging, 28 cases of vertebrobasilar artery dissecting aneurysm were confirmed by digital subtraction angiography. The coincidence rate of high resolution magnetic resonance imaging was 93.02%, significantly higher than 65.12% of the digital subtraction angiography, with statistical difference （P〈0.05）. Conclusions High resolution magnetic resonance imaging can provide lumen information and tube wall information for the lesion site of vertebrobasilar artery dissecting aneurysm with reproducibility and noninvasion, which has important diagnostic value of vertebralbasilar artery dissecting aneurysm.
