空腹血糖联合糖化血红蛋白检测筛查对糖尿病的临床价值

摘    要:目的 分析空腹血糖联合糖化血红蛋白检测筛选糖尿病的临床价值.方法 将460例糖尿病高危人群随机分为观察组和对照组,每组230例,对照组单检测空腹血糖,观察组采用空腹血糖联合糖化血红蛋白检测,患者均行口服葡萄糖耐量实验对检测结果进行检验,比较两组患者的阳性率及准确率.结果 对照组中检测出52例阳性患者,阳性率为22.61%;葡萄糖耐量实验检测出36例阳性患者,准确率为69.23%.观察组中检测出50例阳性患者,阳性率为21.74%;葡萄糖耐量试验检测出49例阳性患者,准确率为98.00%.两组受试者阳性率的比较无统计学差异(P >0.05),准确率的比较差异显著(P<0.05).结论 空腹血糖联合糖化血红蛋白检测的准确性、特异性及灵敏性较高,且操作简便,能及早发现糖尿病可疑患者,具有重要的临床价值,值得推广.

Abstract:Objective To analyze the clinical value of fasting blood glucose (FBG) and glycated hemoglobin (HbAlc) as a screening test for diabetes mellitus. Methods 460 cases of patients at high risk of diabetes were randomly divided into observation group and control group, with 230 cases in each group. The control group was only tested FBG, while the observation group was tested FBG and HbAlc. All patients were treated with oral glucose tolerance test to detect the results, the positive rate and accurate rate of two groups were compared. Results 52 positive patients were detected out in the control group and the positive rate was 22.61%; glucose tolerance test detected out 36 positive patients and the accurate rate was 69.23%. 50 positive patients were detected out in the observation group and the positive rate was 21.74%; glucose tolerance test detected out 49 positive patients and the accurate rate was 98.00%. The positive rate of the two groups had no statistical difference (P 〉0.05), and the accuracy rate of two groups had statistical significant difference (P 〈0.05). Conclusions Fasting blood glucose combined with glycated hemoglobin has high accuracy, specificity, sensitivity and significant clinical value with easy operation, which can detect people with diabetes at an early stage, and deserves clinical application.
