磁敏感加权成像在隐匿性脑血管畸形中的诊断价值

摘    要:目的:探讨磁敏感加权成像(SWI)在隐匿性脑血管畸形中的诊断价值。方法:对52例隐匿性脑血管畸形(海绵状血管瘤27例,静脉血管瘤25例)患者的磁共振影像进行回顾性分析,并比较SWI与其它常规序列对隐匿性血管畸形病灶显示和检出能力的差异。结果:在所有27例海绵状血管瘤病例中,SWI发现病灶数目最多,其它序列发现病灶数目均明显少于SWI;在所有25例静脉血管瘤病例中,SWI较其它常规序列更清晰地显示引流静脉及髓静脉,均呈现特征性的“海蛇头”样表现。结论:SWI能够较常规序列更敏感地发现海绵状血管瘤、静脉血管瘤病变,是诊断隐匿性脑血管畸形的可靠方法,具有很高的临床应用价值,应作为隐匿性血管畸形的常规检查序列。

Abstract:Objective: To evaluate the diagnostic value of susceptibility weighted imaging (SWI) in angiographically occult vascular malformations (AOVM). Methods: The imaging data of MRI were retrospectively analyzed in fifty-two cases(including 27 cases cavernous hemangioma and 25 cases venous augioma). Imaging findings and number of AOVM on SWI was compared with conventional MR sequences. Results: In all cavernous hemangioma cases, SWI detected more cavernous hemangioma lesions than those in other sequences. In all venous angioma cases, the dilated draining veins and medullary veins were shown well on SWI, and assimilated to a "Caput Medusae". Conclusion: SWI could detect lesions of cavernous angioma and venous angioma more sensitively than other sequences, which is a reliable diagnostic technique for AOVM. SWI has higher clinical value, and shouht be the routine sequence in the MRI examination.
