冠状动脉病变与血脂相关性分析

摘    要:目的:探讨冠心病患者冠状动脉病变范围及狭窄程度与血脂的关系。方法:387例选择性冠状动脉造影,252例冠状动脉造影证实冠心病患者按病变范围、狭窄程度分组,术前行TC、TG、LDL-C、HDL-C检查,对比分析冠状动脉病变范围及狭窄程度与TC、TG、LDL-C、HDL-C的关系。结果:1TC、TG、各病变范围及狭窄程度组与正常组均有差异(P<0.05或P<0.01)。2TC各病变组间逐渐升高;3TG1支病变组与3支病变组有差异;LDL-C3支病变组与1、2支病变组有差异;HDL-C1支病变组与2、3支病变组有差异(P<0.01)。4TG不同狭窄程度组均有差异;LDL-C轻度狭窄与重度及闭塞组有差异,中度狭窄组与闭塞组有差异;HDL-C闭塞组与轻、中度狭窄组间有差异(P<0.05或P<0.01)。结论:冠心病患者TG、TC、LDL-C逐渐升高,HDL-C逐渐减低。血脂异常可使脂质在冠状动脉内膜沉积,促进血栓形成,加速冠状动脉粥样硬化的进程,导致冠状动脉狭窄。因此,积极控制血脂对防治冠心病有积极意义。

Correlation analysis of the coronary artery lesion and the blood lipid

Abstract:Objective:To investigate the relationship between the scope and degree of coronary stenosis and the blood lipid.Methods:Three hundred and eighty seven patients undergoing coronary arteriography(CAG) were chosen,of which,252 patients diagnosed as coronary heart disease(CAD) by CAG were divided into different groups according to the extent of length and stenosis of the coronary artery.All patients underwent TC,TG,LDL-C,HDL-C before CAG.Based on the results of CAG,TC,TG,LDL-C and HDL-C,the correlation between the scope and degree of coronary artery stenosis and A TC and TG,LDL-C,HDL-C wasanalyzed.Results:①The differences between TC and TG and LDL-C and HDL-C level and control group were significant(P0.05或P0.01).②The difference level of TC increase gradually.③The differences of TG level was significant between the one and three-vessel lesion groups.The differences of LDL-C level were significant between the one and two-vessel lesion groups and three-vessel lesion group,the differences of HDL-C level were significant between the three and two-vessel lesion groups and one-vessel lesion group(P0.01).④The differences of TG level were significant between all stenosis groups,The differences of LDL-C level were significant between the low stenosis group and severe stenosis and totally occlusion group and between the middle stenosis group and totally occlusion group,The differences of HDL-C level were significant between the totally occlusion group and low and middle stenosis groups(P0.05或P0.01).Conclusion:With the aggravation of scope and stenosis of lesion vessels,the level of HDL-C decrease and TC,TG,LDL-C increase significantly.Dyslipidemia can make lipid deposit in the coronary artery intima,promote thrombopoiesis,accelerate coronary atherosclerotic process,lead to coronary artery stenosis.Therefore,intervening the level of the blood lipid will provide benefit for the prevention of CAD.
