喜炎平联合常规治疗对小儿疱疹性咽峡炎疗效的系统评价

目的 系统评价喜炎平联合常规治疗对小儿疱疹性咽峡炎的疗效及安全性.方法 计算机检索Cochrane图书馆(2011年第2期)、EMBase、PubMed、中国期刊全文数据库(CNKI)、中国生物医学文献数据库(CBM)、中文科技期刊全文数据库(VIP)中(各数据库检索时间均从创建至2011年9月止)关于喜炎平联合常规治疗对小儿疱疹性咽峡炎的随机对照试验(RCT).由2名数据员按纳入与排除标准独立筛选试验,并对纳入研究的方法学质量进行评价,提取资料;用RevMan 5.14软件对数据进行Meta分析.结果 共纳入19篇RCT,包括2 329例患者.喜炎平组总有效率优于对照组RR=1.22,95% CI(1.17,1.26),P<0.000 01],退热时间短于对照组MD=-1.11,95% CI(-1.31,-0.91),P<0.000 01],疱疹消失时间短于常规治疗组MD=-1.42,95% CI(-1.65,-1.18),P<0.00001],差异均有统计学意义.喜炎平组不良反应发生率OR=0.94,95% CI(0.54,1.65),P=0.84]与对照组之间差异无统计学意义.结论 基于现有临床证据,喜炎平联合常规治疗对小儿疱疹性咽峡炎有效,安全性好.但由于纳入研究数量较少,研究质量不统一,结论尚需要更多大样本、高质量临床随机对照试验予以证实.

Systematic Evaluation on Xiyanping Combined Conventional Therapy for Treating Pediatric Herpangina

Objective To systematically evaluate the effects and safety of Xiyanping combined conventional therapy for treating pediatric herpangina.Methods The randomized controlled trials（RCTs） of Xiyanping combined conventional therapy for treating pediatric herpangina were retrieved by the Cochrane Library（Issue 2,2011）,EMbase,PubMed,CBM,CNKI and VIP from their establish to September 2011.Two reviewers independently retrieved RCTs according to the inclusion and exclusion criteria,assessed the methodological quality of included trials,and extracted data.The data analysis was performed by RevMan 5.14 software.Results Nineteen RCTs involving 2 329 patients were ultimately identified.The results of meta-analysis showed that the total effective rate of the Xiyanping group was superior to that of the control groupRR=1.22,95%CI（1.17,1.26）,P〈0.000 01];the fever-reducing time of Xiyanping group was shorter than that of the control groupMD=-1.11,95%CI（-1.31,-0.91）,P〈0.000 01];the extinction time of herpes pharyngis of the Xiyanping group was shorter than that of the control group MD=-1.42,95%CI（-1.65,-1.18）,P〈0.000 01],showing statistical differences among them.There was no statistical difference in the incidence of adverse reactions between two groupsOR=0.94,95%CI（0.54,1.65）,P=0.84].Conclusion Based on the existing clinical evidences,Xiyanping combined conventional therapy is effective for treating pediatric herpangina with good safety.However,due to small number and ununiform quality of included studies,this conclusion needs more large sample size and high-quality RCTs to confirm.
