冠心病合并心力衰竭患者上肢浅静脉血栓发生率横断面调查及影响因素分析

目的调查冠心病(CHD)合并心力衰竭(HF)患者上肢浅静脉血栓(SVT)的发生率,并分析其影响因素。方法选取我院2017年1月至2020年2月收治的743例CHD合并HF患者作为研究对象,收集患者的一般资料及临床资料。统计CHD合并HF患者上肢SVT的发生率,并采用单因素与多因素分析影响CHD合并HF患者上肢SVT发生的因素。结果743例CHD合并HF患者经彩色多普勒超声诊断确诊SVT 376例,确诊率为50.61%(376/743),其中不完全阻塞315例(83.78%),完全阻塞61例(16.22%)。二元Logistic回归分析显示,年龄、静脉给药途径、单次留置针留置时间、LDL和ATⅡ是CHD合并HF患者上肢SVT发生的危险因素,SVT预防性护理是CHD合并HF患者上肢SVT发生的保护因素(P<0.05)。结论CHD合并HF患者上肢SVT的发生率较高,且影响因素较多,治疗过程中应重视对高龄、留置针穿刺、高LDL和高ATⅡ水平患者的筛查与护理,严格按照相关指南与规范控制留置针留置时长,以减少上肢SVT的发生。

Cross section investigation on the incidence of upper limb superficial vein thrombosis and analysis of influencing factors in patients with coronary heart disease and heart failure

Objective To investigate the incidence of upper limb superficial vein thrombosis(SVT)in patients with coronary heart disease(CHD)and heart failure(HF),and analyze the influencing factors.Methods A total of 743 patients with CHD and HF admitted in our hospital from January 2017 to February 2020 were selected as the research objects,and the general data and clinical data of the patients were collected.The incidence of upper limb SVT in patients with CHD and HF was counted,and the factors affecting the occurrence of upper limb SVT in patients with CHD and HF were analyzed by univariate and multivariate methods.Results Among the 743 patients with CHD and HF,376 cases were diagnosed with SVT by color Doppler ultrasound,the diagnosis rate was 50.61%(376/743),among them,315 cases(83.78%)were incomplete obstruction,and 61 cases(16.22%)were completely obstructed.Age,route of intravenous administration,indwelling time of single indwelling needle,LDL and ATⅡwere risk factors of upper limb SVT in patients with CHD and HF,SVT preventive nursing was the protection factors of upper limb SVT in patients with CHD and HF(P<0.05).Conclusion CHD and HF patients have a higher incidence of upper limb SVT,and there are many influencing factors.In the treatment process,attention should be paid to the screening and caring of patients with advanced age,indwelling needle puncture,high LDL and high ATⅡlevels,and strictly control the indwelling time of the indwelling needle follow relevant guidelines and norms,so as to reduce the occurrence of upper limb SVT.
