摘    要:目的比较低通量血液透析(HD)、高通量血液透析(HFHD)和血液灌流与血液透析串联(HP/HD)3种血液净化方式对维持性血液透析患者尿素氮、肌酐和β2-微球蛋白清除的影响。方法将血液净化中心68例维持性血液透析患者随机分成3组,分别是HD组24例,HFHD组22例,HP/HD组22例。在透析前后分别取血检测患者尿素氮、肌酐及β2-微球蛋白。结果3组患者透析前后尿素氮、肌酐均明显下降。HFHD和HP/HD组患者循环中β2-微球蛋白透析显著下降,而HD组透析前后β2-微球蛋白差异无显著性。结论HD、HFHD、HP/HD均能有效清除维持性血液透析患者血液中小分子溶质,而HFHD、HP/HD对β2-微球蛋白清除能力明显优于HD。

Abstract:Objective To compare the effects of different dialysis modes, low - flux hemodialysis(HD), high -flux hemodialysis(HFHD) and hemopeffusion combined hemodialysis(HP/HD) on serum β2-microglobulin. Methods 68 maintenance hemodialysis patients were randomly assigned to low - flux hemodialysis group( HD n = 24), high - flux hemodialysis group ( HFHD n = 22) and hemoperfusion combined hemodialysis group ( HP/HD n = 22) .The serum blood urea nitrogen,creatinine and β2-microglobulin were examined pre and post dialysis. Results The serum blood urea nitrogen, creatinine all reduced through dialysis in three groups. The concentration of serum β2-microglobulin significantly reduced after the dialysis session in the HFHD group and HP/HD group while no sighificance was found in the HD group. Conclusion HD, HFHD and HP/HD all could efficiently eliminate the low molecular weight solutes. While HFHD and HP/HD also could efficiently eliminate the β2-microglobulin in the maintenance hemodialysis patients.
