早期足量胃肠外营养对早产低体重儿出院时体重的影响

摘    要:目的探讨早期足量胃肠外营养对早产低体重儿出院时体重的影响。方法随机将138例符合条件的早产低体重儿分为治疗组及对照组,治疗组早期足量胃肠外营养,对照组应用传统新生儿胃肠外营养。计算两组患儿平均住院时间、生理性体重下降持续平均时间、体重下降幅度及恢复出生体重的平均时间,两组患儿出院时宫外生长发育迟缓(EUGR)的发生率。结果与对照组比较,治疗组患儿平均住院时间缩短,生理性体重下降持续时间缩短,生后体重下降幅度低,出院时EUGR的发生率降低,差异有统计学意义,P〈0.05。结论早期足量胃肠外营养,可降低早产低体重儿EUGR的发生率,提高临床治疗效果,为今后实现追赶性生长打下良好基础,未见明显副作用,临床应用安全有效,值得推广。

Clinical Observation on Leaving-hospital Weights of Preterm Low Birth Weight Infants with Enough Early Parenteral Nutrition

Abstract:Objective To evaluate the influence of early full parenteral nutrition(PN)on the weight gaining of premature low-birth-weight infants(LBWIS)at discharge.Methods One hundred and thirty-eight eligible premature LBWIS were randomly divided into two groups(69 cases each group).Early full PN was given to therapy group,and traditional PN was used in control group.The average length of stay in hospital,average duration and range of physiological weight drop,and the incidences of extrauterine growth restriction(EUGR)at discharge were analyzed.Results In therapy group,the time of hospitalization and birth weight regain were significantly shorter than those of in the control group(P〈0.05),the average duration and range of physiological weight drop in therapy group were also less than those of in the control group significantly(P〈0.05),the incidence of EUGR at discharge was significantly lower than the control group(P〈0.05).Conclusion Early full PN is able to reduce the incidence of EUGR of incidence of premature LBWIS and this will provide good basis for catching up growth.There is no side effect noted.
