摘    要:目的探讨重症监护病房COPD患者机械通气时间的因素。方法选取重症监护病房进行机械通气治疗的54例COPD患者为研究对象,根据机械通气时间分为A组(≤7d组)18例、B组(8~14 d组)18例和C组(>14 d组)18例,后将三组人员中的不同插管前和插管后因素进行比较。结果 C组年龄大于A组及B组,心率高于A组及B组,有脑梗死病史高于A组及B组,血清白蛋白、前白蛋白均低于A组及B组,插管后消化道出血、血压降低及呼吸机相关性肺炎均高于A组及B组,P均<0.05,而三组伴有与不伴有糖尿病者无显著性差异,P>0.05。结论重症监护病房COPD患者机械通气时间的相关因素主要包括插管前和插管后相关因素,应引起的重视。

Abstract:Objective To study the related factors of mechanical ventilation time of patients with chronic obstructive puhnonary disease (COPD) in intensive care unit (1CU). Methods 54 patients with COPD in ICU were divided into three groups based on ventila- tion time. The ventilation time of the group A ( 18 cases) was less than or equal 7 days. The ventilation time of the group B ( 18 cases) was among 8 to 14 days, and the ventilation time of the group C ( 18 cases ) was more than 14 days. The factors before and after cannula were compared among the three groups. Results The average age of the group C was the oldest among the three groups. The heart rate was higher in the group C than in the group A and B. The patients in the group C had the most history of cerebral infarction, and the levels of serum albumin and prealbumin were the lowest in the group C. The incidence of digestive tract hemorrhage, blood pressure lowering and ventilator associated pneumonia after eannula was higher in the group C than in the group A and B (P 〈 0.05 ). There was no significant difference of patients complicated with diabetes mellitus ( P 〉 0.05 ). Conclusion The related factors of mechanical ventilation time of COPD patients in ICU mainly include factors betbre and after cannuta, and they should be paid enough attention.
