夜间氧疗联合噻托溴铵对老年COPD呼吸衰竭患者肺功能和生活质量的影响

目的探讨夜间氧疗联合噻托溴铵对老年慢性阻塞性肺疾病(COPD)呼吸衰竭患者肺功能和生活质量的影响。方法将130例老年COPD呼吸衰竭患者随机分入对照组与观察组,两组患者均接受常规治疗及夜间氧疗,观察组在上述治疗基础上同时给予噻托溴铵吸入治疗,疗程12周。比较两组治疗前后6分钟步行距离(6MWD)、改良医学研究学会呼吸困难量表(MMRC)、肺功能及生活质量的差别。结果治疗后观察组6MWD及MMRC评分均显著优于对照组(P<005);观察组治疗后肺功能指标FEV1及FVC显著提高,与对照组相比差别具有统计学意义(P<0.05);观察组患者治疗后生活质量圣.乔治呼吸问卷评分(SGRQ)显著优于对照组(P<0.05)。结论夜间氧疗联合噻托溴铵可显著改善老年COPD呼吸衰竭患者肺功能和生活质量。

Effect of night oxygen therapy combined with tiotropium on lung function and life quality in elderly COPD complicated with respiratory failure

Objective To explore the effect of night oxygen therapy combined with tiotropium on lung function and life quality in elderly COPD complicated with respiratory failure. Methods 130 elderly cases with COPD complicated with respiratory failure were randomly divided into control group and experience group. Two groups received routine treatment, meanwhile, experience group were given inhaled tiotropium. A course of treatment was 12 weeks. 6MWD, MMRC scores, lung function and life quality were compared between two groups. Results 6MWD and MMRC after treatment in experience group were superior to control group （ P 〈 0. 05 ） ; Comparison with control group, FEV, and FVC after therapy in experience group greatly improved （P 〈0. 05） ; SGRQ scores after intervention in experi- ence group were much lower than those in control group （ P 〈 0.05 ）. Conclusions Night oxygen therapy combined with tiotropium for elderly COPD complicated with respiratory failure greatly improve lung function and life quality.
