老年血糖异常患者血清胆红素水平变化及危险因素分析

目的研究老年血糖异常患者血清胆红素的变化及影响血糖水平的危险因素。方法纳入住院老年患者644例,其中血糖异常患者303例(血糖异常组),血糖正常患者341例(对照组)。记录所有研究对象的一般情况及各项指标,包括胆红素、肝功能、血脂、C反应蛋白(CRP)、脑钠肽(BNP)等,进行相应统计学分析。结果与对照组相比,血糖异常组患者血清总胆红素(TB)、间接胆红素(IB)水平降低(P0.05),血清CRP水平升高(P0.01)。按血清总胆红素四分位数分组,胆红素高水平组比胆红素低水平组血糖、血压水平异常的百分比低(P=0.000,P=0.016)。有无血糖异常及血糖异常的严重程度与血清TB、IB水平呈弱负相关(P0.05)。结论在老年人群中,生理范围内轻度升高的胆红素可能是血糖异常的一个保护因素。

Investigation on serum bilirubin levels and corresponding risk factors in elderly patients with abnormal glucose levels

Objective To investigate the bilirubin levels in elderly patients with abnormal glucose levels and its risk factors. Methods 644 hospitalized elderly patients were enrolled. Among them,303 elderly patients with abnormal serum glucose levels were selected as the experimental group and 341 elderly patients with normal serum glucose levels were selected as the control group. The basic characters,including serum bilirubin levels,hepatic enzyme levels,lipid levels,CRP,BNP were all collected. Results In the experimental group,total bilirubin （TB） and indirect bilirubin （IB） levels were lower （P〈 0.05） and serum CRP levels were higher （P 〈 0.01） when compared with the control group. Grouped by the first quartile,higher-TB-level group had higher percentage of patients with abnormal blood pressure or abnormal serum glucose （P = 0.000,P = 0.016）,when compared with lower-TB-level group. In logistic regression analysis,hypertension and dislipidemia were both the independent risk factors for the abnormality in glucose metabolism （P 〈 0.05 ）. Conclusion In elderly people,the slightly elevated serum bilirubin levels within the normal range are the protective factors for the glucose metabolism.
