摘    要:目的探讨多层螺旋CT(MSCT)三维重建在慢性化脓性中耳炎中的应用价值。方法使用MSCT对正常30耳,慢性化脓性中耳炎41耳进行1.25 mm/4i薄层扫描,把扫描所得的容积数据用容积重现法(VR)进行三维重建,记录重建结果与术中所见进行对比分析。结果VR法能对正常颞骨内结构,如听骨链、面神经管、耳蜗、前庭、半规管等均显示满意,对外侧半规管淋巴漏的诊断和手术所见完全相符,对胆脂瘤范围和锤骨、砧骨破坏的诊断和手术所见基本相符。结论VR三维重建法可以为临床提供详细、立体的影像资料,对明确诊断、制定手术方案和提高手术安全性具有重要意义。

Abstract:Objective To evaluate the applied value of multislice spiral CT3D reconstruction in the chronic suppurative otitis media(CSOM).Methods Forty one ears with CSOM and thirty normal ears were performed multislice spiral CT(MSCT)with thin-slice(1.25mm/4i).The volume data was transferred to graphic workstation and the structures in temporal bone were reconstructed by using volume render(VR).Results 3D reconstruction can demonstrate almost all structures in the temporal bone,such as the ossicles,the facial nerve canal,the cochlea,the vestibule,the semicircular canal,etc.The perilymphatic fistula of semicircular canal showed in3D image accorded with the finding of operation.Most of erosion of the ossicles and the extension of the cholesteatoma showed in3D image were in consistent with the finding of operation.Conclusions 3D reconstruction offers improved morphological analysis,and will benefit diagnosis,surgical planning.
