| 摘 要： | 目的:总结腹腔镜手术中处理胆管结石的临床经验,探讨手术方法的选择及应用注意事项。方法:分别采用腹腔镜经胆囊管胆总管探查取石、腹腔镜胆总管切开探查一期缝合、腹腔镜胆总管切开探查T管引流及腹腔镜胆囊切除术(LC)联合术前或术后内镜下十二指肠乳头切开取石4种方法。结果:4种方法均取得满意结果;腹腔镜胆总管切开探查一期缝合胆漏的发生率较高。结论:腹腔镜手术中处理胆管结石并没有固定的原则可以遵循,需要术者根据患者的具体情况,自身的经验水平以及相关的设备条件,做出最适合患者的判断。

| Abstract: | Objective:To summarize the clinical experiences of treating choledocholithiasis in laparoscopic operation,discuss the choice of operating methods and announcements in practice.Methods:Analyze the clinical data of 84 patients,performing laparoscopic common bile duct exploration(LCBDE)with transcystic approach,LCBDE with primary suture,LCBDE with T tube drainage and laparoscopic cholecystectomy associating with preoperative or postoperative endoscopic sphincterotomy(ES)respectively.Results:All the four operation methods got satisfied results,neverthless the rate of bile leakage was higher in LCBDE with primary suture.Conclusions:There are no fixed rules can be complied when treating choledochoithiasis with laparoscopic operation.The optimal judgment made by the perator depened on the situation of the patient,the experiences of the operator and the equipments related. |
