老年人肺结核的影像学表现

目的:探讨老年肺结核的影像学表现特点,提高老年肺结核的诊断水平.方法:男102例,女20例,年龄60～89岁,全部患者均摄有胸部平片,其中49人进行了CT检查,13例增强.结果:122例中结核病灶在3个肺野以上的达79例(64.8%).以浸润增殖为主74例(60.7%);以纤维化病灶为主要表现者70例(57.38%).空洞45例(36.89%),结核球12例(9.68%),胸膜病变68例(56.22%),CT发现肺门淋巴结肿大26例,纵隔淋巴结肿大33例,27例有淋巴结钙化.43例经纤维支气管镜检查证实,痰菌检查证实20例,其余59例经临床表现和治疗随访中证实.结论:老年人肺结核病灶性质在影像学表现以空洞和广泛纤维化为特点,CT检查优于平片.

The imaging manifestation of pulmonary tuberculosis in elder patients

Objective To study the features of X-ray and CT with pulmonary tuberculosis and try to improve the diagnosis level.Methods The series of patients included 102 male and 89 female,with an age ranging from 36 to 73 years old X-ray plain film were performed in all cases,and CT in 49 cases,13 cases had enhanced CT.Results Invovement of three lung field in 122 cases(64.8%),There were 74 cases of invasion and proliferation of lung(60.7%),70 cases of fibrosis(57.4%),cavition 45 cases(36.89%),tuberculoma(9.68%),pleural lesions(56.22 %),There were 26 cases of pulmonary hilar and 33 cases of mediastinal lymphnode enlargement,with 27 cases of calcification enlargement through CT examination.43 patients were proved by fibroscopic examination and 20 patients tuberculosis bacteriology check.In other 59 cases are proved by clinical presentation and delayed treatment.Conclusion The features in elder patients were cavitations and excessive tuberculofibrosis.CT is superior to X ray plain film in diagnosis of pulmonary tuberculosis in elder patients
