激素性股骨头缺血坏死血小板检测及其意义

初步分析激素性股骨头缺血坏死与系统凝血状况的相互关系,进一步探讨激素导致股骨头缺血坏死的发病机理。检测21例激素性股骨头缺血坏死患者(A组)血小板总数,并同随机抽取的10例创伤性股骨头缺血坏死患者(B组)及20例健康人群体(C组)血小板总数做对比分析。结果显示A组中,有11例血小板总数低于正常参考值水平,而B、C组血小板总数均在正常参考值范围;A组血小板总数与B、C组血小板总数对比均有显著性差异(P<0.01)。认为激素性股骨头缺血坏死发病过程中,系统存在异常的凝血状态。这种异常的凝血状态很可能是外源性激素造成了骨内外血管内皮损害或免疫复合物沉积在血管壁,进一步激活了血小板及其它凝血活性物质后引起的。

Platelet Detection and Its Significance in Steroid-induced Necrosis of the Femoral Head

The interrelation of systemic coagulation and steroid - induced necrosis of the femoral head was analysed and the mechanism of steroid - induced necrosis of femoral head was studied. The platelets of 21 patients with steroid - induced necrosis of femoral head (group A) were detected, and it was comparded with that of 10 patients with traumatic necrosis of the femoral head (group B) and 20 healthy subjects (group C) . Results showed that in group A, the platelet levels in 11 patients was lower than normal, and the platelet levels in group A significantly decreased than that of in group B and in group C (P<0.01) .The authors consider that is abnormal coagulation in steroid - induced necrosis of femoral head, and it might be caused by exogenous steroid damaged the vascular endotheliod of those intraosseous and extraosseous; or the platelet and other coagulable active mass be actived by immunocomplex deposited in the vascular wall.
