锥形束CT在下颌骨髁突骨折治疗中应用价值的探讨

目的：探讨口腔颌面锥形束CT( CBCT)在下颌骨髁突骨折治疗中的临床应用价值。方法回顾分析自2013年8月~2014年8月诊治的成年髁突骨折患者30例(33侧),30例患者均行CBCT检查,通过CBCT影像评估髁突骨折类型及骨折移位情况,选择手术或保守治疗,治疗后7天、4周、3个月的追踪随访并评估临床效果。结果在30例患者中,根据CBCT影像诊断及分型,14例(15侧)有手术适应证,其中12例(12侧)选择了手术治疗,术中情况与术前评估基本一致,2例(3侧)放弃手术治疗；16例(18侧)非手术适应证,均选择了保守治疗。两种方法均在治疗后7天、4周、3个月进行随访并观察治疗效果。结论 CBCT在髁突骨折的诊治中简便可靠,对髁突骨折的诊断分型及治疗选择具有较高的临床价值。

The use of CBCT in the diagnosis and treatment of condylar fractures

Objective To evaluate the clinical application of cone beam CT ( CBCT) in the treatment of condylar fracture. Methods Thirty cases of adult condylar fracture (33 sides) were included in this study. The condylar fracture type and displacement were assessed by CBCT. Surgery or conservative treatment was chosen based on the fracture types. The patients were followed up 7 days, 4 weeks and 3 months after treatment. Results Among the 30 patients, based on diagnosis and classification by CBCT, 14 cases (15 sides)had indications for operation and 12 were treated by surgery and two patients(3 sides) gave up surgery. Sixteen cases (18 sides) did not have surgical indication and chose conservative treatment. The treatment outcome of the two groups was evaluated 7 days, 4 weeks and 3 months after treatment. Conclusion The CBCT is very helpful in the diagnosis and classification of condylar fracture and in the treatment option.
