经阴道彩色多普勒超声对宫角妊娠的超声诊断与分型

【目的】探讨经阴道彩色多普勒超声对宫角妊娠的超声诊断价值。【方法】回顾性分析本院经手术及病理证实的55例宫角妊娠经阴道彩色多普勒超声诊断结果并进行分型。【结果】55例宫角妊娠患者中，总的诊断符合率为83．6％，根据其声像图特点，分为三型，胚囊型30例，不均质包块型20例，破裂型5例，诊断符合率分别为93．3%（28／30），75％（15／20），60％（3／5）。在胚囊型中，误诊为宫内妊娠2例，不均质包块型中，误诊为间质部妊娠3例，误诊为滋养细胞肿瘤2例。破裂型中误诊为宫外孕2例。【结论】经阴道彩色多普勒超声对宫角妊娠的超声诊断与分型，可以提高超声诊断符合率，具有一定的临床应用价值。

Application of Transvaginal Color Doppler Sonography in Diagnosis and Typing of Cornual Pregnancy

Objective] To explore the value of transvaginal color doppler sonography （TV-CDS） in the diagnosis of cornual pregnancy. Methods] The ultrosonographie features of 55 cases with eornual pregnancy confirmed by operation and pathological results in our hospital were reviewed retrospectively and typed. Methods]According to the ultrasonic image findings, cornual pregnancy included three types including embryo sac type（30/55）, heterogeneous mass type（20/55） and ruptured type（5/55）. The diagnostic accordance rate of three types was 93.3% （28/30）, 75% （15/20） and 60% （3/5）, respectively. Two intrauterine pregnancies in the embryo sac type, 3 intramural pregnancy and 2 trophoblastic tumors in heterogeneous mass type and 2 ectopic pregnancies in the ruptured type were misdiagnosed. Conclusion] TV CDS has high diagnostic accordance rate and can be used as routine examination of cornual pregnancy.
