不同年龄大肠息肉特点及内镜下治疗的疗效

目的 探讨不同年龄大肠息肉的特点及内镜下治疗的方法与临床效果.方法 将2 623例行内镜检查患者按年龄段分为2组：中青年组（＜60岁）1 236例,老年组（≥60岁）l 387例.比较2组大肠息肉检出率、癌变率及大肠息肉内镜下特点与病理类型,并观察内镜下治疗大肠息肉的临床效果.结果 发现大肠息肉患者417例（共567枚息肉）,均经内镜下手术治疗成功,成功率为100％,术中无严重出血、穿孔、烧灼伤等并发症发生.老年组的大肠息肉检出率和癌变率均显著高于中青年组（21.77％比9.30％、3.31％比1.74％,均P＜0.05）,且息肉越大,癌变率越高（P＜0.05）.中青年组以炎性息肉为主,老年组以腺瘤性息肉为主;2组息肉分布均以直肠和乙状结肠为主.结论 内镜检查是发现并处理大肠息肉的有效方法.内镜下大肠息肉切除术具有简单、创伤小、痛苦少、省时、安全、费用少、恢复快等优点.

Characteristics and Endoscopic Treatment of Colorectal Polyps in Patients of Different Ages

divided into two groups： younger group （age〈60 years old, n=l 236）and elderly group （age i〉60 years old, n=1387）. Detection rate, eanceration rate, endoscopic features, pathological types and therapeutic outcome of colorectal polyps were compared between the two groups. Results A total of 417 cases of colorectal polyps（567 polyps） were found and all patients were successfully treated by endoscopic surg ery. No serious bleeding, perforation, burns and other complications occurred during operation. Compared with younger group, detection rate and canceration rate of colorectal polyps significantly increased in elder group（21.77% vs 9.30% and 3.31% vs 1.74%, respectively; P〈0.05）. Furthermore, eaneeration rate was positively correlated with polyp size（P〈0.05）. Polyps were mainly located in the rectum and sigmoid colon. However, inflammatory polyps were mainly distributed in younger group but adenomatous polyps in elderly group. Conclusion The colonoscopy is an effective measure to detect colorectal polyps. The polypeetomy is a simple, timesaving, economical and safe procedure for the treatment of colorectal polyps with the advantages of small trauma, less pain and rapid recovery.
