原发性胆囊癌的超声诊断与误诊分析

目的 根据原发性胆囊癌的超声声像改变特征进行分析并探讨误诊原因。方法 ２４例原发性胆囊癌患者均经手术与病理证实并对其声像特征进行回顾性分析。结果 其声像特征可分成四型：胆囊壁增厚型１３例，息肉样病变４例，实块型６型，胆囊体积增大１例。超声诊断正确率４５．８％，１３例误诊为慢性胆囊炎，胆炎结石，误诊率５４．２％。结论 二维超声有助于原发性胆囊癌的诊断，为避免误诊应仔细观察原发性胆囊癌的声像改并仔细进

THEPRELIMINARY ANALYSIS OF ULTRASONOGRAPHIC FEATURES AND MISDIAGNOSTIC CAUSES OF PRIMARYGALLBLADDER CARCINOMA

Objective To analyze the ultrasonographic features of primary gallbladder carcinoma and discuss the causes of misdiagnosis.Methods 24 patients of primary gallbladder carcinoma,verified by surgery and histopathological examinations,were analyzed retrospectively by the 2diamensional ultrasonographic features.They were divided into 4 types according to the sonographical characteristics.Results Type 1:Increase of thickness of gallbladder wall 14 cases,Type 2:Polypoid leision 4 cases,Type 3:Solid mass 6 cases and Type 4:Big gallbladder 1 case.The diagnosis was correct in 458 % and 13 cases were misdiagnosed as chronic choleycystitis with gallstones(542%).Conclusion The 2 dimensional ultrasonography is much helpful in the diagnosis of primary gallbladder carcinoma.In order to reduce the rate of misdiagnosis,one should examine patients carefully and pay close attention to the changes of sonographic features in patients with primary gallbladder carcinoma.
