糖尿病人脑血管血流动力学指标的变化特征

目的：比较和分析糖尿病患者和对照组脑血管血流动力学指标（CVHI）差异和变化特征。方法：以整群抽样的方法抽取上海市南汇区3个镇35岁及以上人群进行糖尿病现患调查，并进行CVHI检测，对检测指标进行统一积分，以1：3的比例根据年龄和性别在人群中选择非糖尿病患者作为对照组，比较和分析两组人群CVHI各检测指标和总积分的差异。结果：糖尿病人的平均血流速度（Vmean)、最小血流速度（Vmin)和总积分显著低于非糖尿病人（P＜0．01），而脉搏波波速（WV），外周阻力9RV）、特异性阻抗（Zcv)、动态阻力9DR）和临界压水平（CP）等指标则显著高于非糖尿病人，各单项检测指标中变化幅度较大的指标分别为DR、RV、Vmin、WV和Zcv。结论：糖尿病人的脑血管血流动力学检测指标存在功能异常，主要表现为总积分和最小血流速度下降，外周阻力、特性阻抗、脉搏波波速和动态阻力升高。

The Characteristics of Cerebral Vascular Hemodynamic Indexes in Diabetic Patients

Objective To compare and analyze the difference and variance of cerebral vascular hemodynamic indexes (CVHI) between diabeties patients and controls. Methods A cross sectional survey on diabetes was carried out and CVHI indexes were checked among people aged 35 years and over in 3 towns in Nanhui District of Shanghai City. The diabetes patients in the population were defined as case group and controls without diabetes were selected from the same population and matched with age and sex according to case group.Differences of CVHI indexes between two groups were compared. Results V mean , V min and accumulative score of CVHI indexes in case group were significantly lower than those in controls. However, WV, RV,Z cv , DR and CP were significantly higher than those in control group. Differences of DR, RV, V min , WV and Z cv between two groups were relatively higher. Conclusion Abnormality of CVHI indexes in diabetes patients includes decreases of V mean , V min and accumulative score and increases of WV, RV, Z cv , DR and CP.
