摘    要:目的:探讨sTREM-1在新生儿呼吸机相关性肺炎(VAP)中的作用。方法:将我院新生儿科2008年10月~2011年10月间收治NICU应用气管插管进行机械通气的患儿168例分为2组,实验组中属于并发VAP的患儿共32例,对照组中属于未并发VAP的患儿共136例,动态观察以上两组肺泡灌洗液中sTREM-1通过呼吸机治疗前后的变化。结果:对照组随着病情的好转,肺泡灌洗液中sTREM-1水平逐渐降低;而实验组随着肺炎的发生,肺泡灌洗液中sTREM-1水平有所升高,在对感染加以控制之后,水平逐渐下降。结论:测定肺泡灌洗液中sTREM-1水平,为客观早期的判断VAP的发生及病情进展提供参考指标。

The Clinical Significance of Detecting the Level of Bronchoalveolar Lavage Fluid of Strem-1 in Ventilator-Associated Pneumonia

Abstract:Objective:To investigate the role of sTREM-1 in the neonatal ventilator-associated pneumonia.Methods:Will be our hospital in 2008 from 10 months to October 2011 admitted to the NICU application of endotracheal intubation for mechanical ventilation in children with 168 cases divided into two groups,the experimental group for concurrent VAP 32 cases,and the control group for not concurrent VA P 136 examples,the dynamic observation of lavage fluid in ventilator before and after treatment.Results:In the control group as the condition improves,the lavage fluid of sTREM-1 level decreased;experimental group with the occurrence of ventilator-associated pneumonia,the bronchoalveolar lavage fluid of sTREM-1 level increased,With the control of infection,the level gradually decreased.Conclusion:The determination of sTREM-1 levels in the bronchoalveolar lavage fluid,provide a reference for objective early to judge the incidence of VAP and progression indicators.
