血缘性活体部分肝移植术:附1例报告

目的报告1例血缘性活体部分肝移植术的临床体会。方法受体为12岁女童,诊断为先天性肝纤维化,供体为患儿的父亲,手术方式为切取供体肝脏的左半肝,并原位移植给患儿。术后进行了严格的监护治疗。结果供肝手术历时540分钟,切取肝脏450g,术后恢复顺利。受体手术历时685分钟,出血1400ml,无肝期80分钟,移植肝脏热缺血时间为0,冷缺血时间30分钟。术后采用FK506、晓悉及皮质激素行免疫抑制治疗,术后2月各项生化指标正常。结论活体部分肝移植术技术要求高。良好的肝外科及血管外科技术及术后管理是确保肝移植术成功的关键。

Living-donor liver transplantation: one case report

Objective To sumarize the clinical experience of one case living-donor liver transplantation.Methods The recipient was a 12 years old girl with congenital hepatic fibrosis. The left lobe of the liver (segment II,III and IV) from her father was excised and was oethotopically transplanted to the child. Postoperative intensive care was carried out seriously.Results The resection of left lobe (450g) of liver from donor took 540 minutes. The operating time of graft implantation was 685 minutes. The blood loss was 1400 ml. Ahepatic stage was 80 minutes. The hot graft ischemic time was 0 minutes and cold ischemic time was 30 minutes. FK506,mycophenolate mofetil (MMF) and prednisone were used for postoperative immunosuppression. The biochemical parameters of the child became to normal in 2 months and had a good quality of life till now.Conclusion Living donor liver transplantation is a complex operation. Perfect operative technique in hepatic and vascular surgery and postoperative intensive care are the keys to ensure the success of the procedure.
