64排螺旋CT对肺部空洞性病变的诊断价值

目的回顾性分析肺部空洞性病变的64排CT影像学特征,以提高诊断及鉴别诊断水平。方法 38例经病理或经治疗证实的空洞性病变,全部病例均进行了64排CT平扫,21例同时有CT增强扫描。结果 32例为单发结节空洞,6例为多发空洞。8例炎性空洞中,主要为厚壁空洞,很少或无分叶,模糊征阳性7例,4例有胸膜增厚、粘连,1例有较典型的胸膜凹陷征。20例癌性空洞中,4例为转移性癌性空洞。16原发癌性空洞及2例转移性癌性空洞外缘均有分叶、边缘不规则和毛刺征,大多病灶均有胸膜凹陷征;2例转移性癌性空洞外缘光滑,内缘可见壁结节,行CT增强扫描,CT值提高幅度超过20HU。10例为结核性空洞,很少或无分叶征象,7例为厚壁空洞,4例空洞内外壁均毛糙不光滑,很少强化,所有病灶均可见卫星灶,4例胸膜凹陷征阳性。结论利用64排CT多平面成像的特点,可以多方位详细观察空洞内部结构、外缘分叶的深浅、周围特征及空洞壁强化特征,为病变的诊断及鉴别诊断提供重要的诊断依据。

Value of 64-slice spiral CT scanning in diagnosis of pulmonary cavity
