苦参素、左旋咪唑治疗慢性乙型肝炎临床观察

目的 :探讨苦参素注射液、左旋咪唑涂布剂单独及联合应用治疗慢性乙型肝炎的临床疗效。方法 :6 3例慢性乙型肝炎病毒 (HBV)感染者随机分为治疗A组、B组、C组和对照D组。治疗A组给予苦参素注射液 4 0 0～ 6 0 0mg ,每天 1次肌肉注射 ;B组给予左旋咪唑涂布剂 5 0 0mg ,每周 2次涂于腿部皮肤 ;C组为两药联用 ;对照D组给予维生素及一般护肝药物。疗程均为 3个月。结果 :各组血清丙氨酸转氨酶 (ALT)及血清总胆红素 (TBIL)复常率间差异均无显著性 (P >0 .0 5 )。治疗第 1、2个月末各组乙型肝炎病毒血清标志物改变差异均无显著性 ,但治疗结束时治疗A、B、C组血清HBeAg阴转率分别为 33.3%、30 .0 %、38 .9% ,HBVDNA阴转率分别为 4 6 .2 %、4 1.7%、5 5 .0 % ,均高于对照组 (P <0 .0 5 )。结论 :苦参素注射液有抑制病毒复制、调节免疫、抗感染作用 ,与左旋咪唑涂布剂联合应用优于单用 ,可成为治疗慢性乙型肝炎安全、经济、方便而有效的药物。

Treatment with matrine and/or levamisole in chronic hepatitis B

Objective:To study the efficacy of matrine injection and/or levamisole liniment(L.L.) in treatment of chronic hepatitis B.Methods:Thirty eight patients were divided at random into 3 treatment group A,B,C and a control group.Treatment group A was given matrine injection 400 to 600 mg/d intramuscularly,group B L.L. 500 mg(5 ml) percutaneously twice a week,group C 2 drugs in combination,and the control group the general liver protection drug,vitamins.The therapeutic course was 3 months.Results:The normalization rates of serum alanine amino transfernase(ALT) and total bilirubin(TBIL) of each of the 3 treatment groups and the control group were not different significantly( P >0.05) after treatment.The negative conversion rates of there groups were not different significantly after 1 month and 2 months treatment,but after 3 months treatment,the negative conversion rate of serum HBeAg of the treatment group A,B,C were 33.3%,30.0%,38.9%,HBV DNA of the treatment group A,B,C were 46.2%,47.1%,55.0%,significantly higher than that of the control group( P <0.05).Conclusions:Matrine injection is effective in inhibiting proliferation of HBV,regulating immune reaction of the host and antagonisting liver inflammetion,so the use of matrine injection in combination with L.L. is a safe,economic,convenient and effective durg in treatment of chronic hepatitis B.
