老年人上消化道大出血——122例报告

本文总结了我院1985年1月至1990年1月收治的122例老年人上消化道大出血患者的病因、出血部位及出血程度等方面的特点。发现老年人胃溃疡并出血者比例明显增高,高位胃溃疡并出血者较国内文献报告亦高。其中发现2例Dieulafoy溃疡。老年人易发生失血性休克,本组发生休克者32例。80岁以上,就诊时已发生休克,血色素低于7g的食管胃底静脉曲张破裂出血者死亡率明显升高。急诊内镜对老年患者只要选择好适应症,术前作好抢救准备,操作熟练轻巧,仍不失为安全、可靠的检查手段。本组70岁以上40例经检查后未发生任何并发症。

Massive Upper Gastrointestinal Bleeding in Older Patients:A Report of 122 Cases

Our paper summarized the. data of 122 older patients with massive upper gastrointestinal bleeding from Jan, 1985 to Jan, 1990, including etiology, bleeding spot and bleeding grade. We found the high bleeding rate in older patients with gastric ulcer, especially proximal gastric ulcer, and found two cases of Dieulofoy ulcer. The older patients were easy to have hemorrhagic shock. There were 32 cases in our data. We also found high mortality in patients older than 80 who have esophageal varices, shock and hemoglobin lower than 70 gram/L. Emergent endoscopy was safe, reliable step in older patients, provided by control the indications, preparat ons of preoperation and the manipulation. No complications were found in 40 patients above 70 years old who perform endoscopy.
