肩关节扩张造影的应用价值

目的 探讨肩关节扩张造影对肩部疾病的诊断和治疗价值。方法 对19例肩关节疾病病人，进行肩关节扩张造影。观察扩张造影前后关节腔内的显影变化。结果造影显示加压扩张前，19例病人肩关节腔平均容积为6.5ml，加压扩张至关节囊破裂时，平均容积增加至12．5ml。19例病人经治疗后随访11～16个月，16例疗效为优，3例为良。结论 肩关节扩张造影既可用于冻结肩的诊断，同时也可对其进行有效的治疗。它还可以弥补常规肩关节造影对于肩袖破裂造成漏诊的缺陷。

Evalucation for Distension Arthrography of Shoulder

Objective To investigate the value of shoulder distension arthrography.Methods When correct needle placement was established,contrast medium was slowly injected into 19 cases until resistance was offered.The changes of joint capacity were observed.Results Decreased joint capacity was observed(an average of 6.5ml).When joint capacity increased to an average of 12.5ml,the capsule ruptured in the wall of the subscapular bursa or in bicipital sheath.All the 19 patients were followed up for 11 to 16 months,and excellent results in 16 cases,good in 3.Conclusion Distension arthrography was reliable and highly effective in diagnosis and treatment of frozen shoulder.In addition,it would be more effective than routine arthrography in diagnosis of rotator cuff injury.
