小儿十二指肠膜式狭窄的超声诊断价值

目的总结小儿十二指肠膜式狭窄（MDS）术前超声声像图特点。 方法对2004—2017年经首都医科大学附属北京儿童医院超声检查、上消化道造影和临床手术检查诊断为十二指肠膜式狭窄94例患儿的超声声像图特征进行总结。 结果94例MDS患儿术前超声显示十二指肠膜式狭窄81例（86.2%，81/94）、肠腔内隔膜65例（69.1%，65/94），其中50.8%（33/65）可观察到孔隙。依据超声检查及上消化道造影（42.9%，18/42）检查结果，94例患儿均由临床进行了剖腹手术或腹腔镜下纵切横缝隔膜切除术，术中可见隔膜及十二指肠梗阻。 结论超声检查可清晰显示肠梗阻位置并观察到肠腔内隔膜，对手术方式的选择有重要意义，是术前判断十二指肠膜式狭窄首选的检查方法。

The value of ultrasonographic diagnosis of membranous duodenal stenosis in children

ObjectiveTo summarize the preoperative ultrasonic characteristics of membranous duodenal stenosis (MDS) in children. MethodsThe ultrasonographic features of 94 children with operation-confirmed MDS were summarized, who underwent ultrasonography and upper gastrointestinal contrast at Beijing Children's Hospital Affiliated to Capital Medical University from 2004 to 2017. ResultsPreoperative ultrasound in 94 patients with MDS showed 81 cases (86.2%, 81/94) of MDS and 65 cases (69.1%, 65/94) of intestinal septum. Of them, the pores were observed in 50.8% (33/65) cases. In all 94 patients septum and duodenal obstruction were confirmed by operation. ConclusionsUltrasonography can clearly show the location of intestinal obstruction and identify the septum in the intestine. Ultrasound is helpful in surgical planning, and is the method of choice for the preoperative diagnosis of MDS.
