会阴无保护分娩的临床效果及安全性

目的：探讨会阴无保护分娩的临床效果及安全性。方法随机选取奉化市妇幼保健院2012年4月至2014年5月126例正常分娩产妇作为研究对象，观察组63例产妇采用无会阴保护状态下分娩，对照组63例产妇采用传统托肛会阴保护法状态下分娩，比较两组临床疗效及安全性。结果观察组产妇会阴完整性比率为22．22％，高于对照组，而Ⅰ级裂伤和Ⅱ级裂伤比率分别为73．01％和4．76％，组间差异有统计学意义（Z＝2．045，P＜0．05），会阴侧切的比率为12．69％，低于对照组，组间差异有统计学意义（χ2＝5．704，P＜0．05）；观察组产妇会阴部疼痛程度分布的差异具有统计学意义（Z＝2．304，P＜0．05），观察组产妇满意度为95．23％，明显优于对照组，组间存在统计学差异（χ2＝5．143，P＜0．05）。结论会阴无保护分娩简单实用，且很大程度常降低会阴侧切率，有效保护和减少了会阴损伤，减轻产妇伤口疼痛及会阴侧切术对生活的影响，从而提高初产妇的生活质量，值得临床进一步推广。

Clinical effect and safety of unprotected perineum delivery

Objective To explore the clinical effect and safety of unprotected perineum delivery .Methods In Fenghua Municipal Maternal and Child Health Care Hospital during the period of April 2012 to May 2014 totally 126 cases of normal pregnant women were randomly selected as research objects .There were 63 cases in the observation group accepting unprotected perineum delivery and 63 cases in control group accepting traditional supporting anus perineum protection in delivery .Two groups were compared in clinical effect and safety . Results In the observation group the perineal integrality rate was 22.22%, which was higher than that in the control group .The rate of grade I laceration and grade II was 73.01%and 4.76%, respectively, and the difference between two groups was significant (Z=2.045, P<0.05).The perineal episiotomy rate was 12.69% in the observation group, which was lower than that in the control group (χ2 =5.704,P<0.05), and the distribution of perineal pain degree was statistically different (Z=2.304, P<0.05).The satisfaction degree of cases in the observation group was 95.23%, which was obviously higher than that in the control group (χ2 =5.143, P<0.05). Conclusion Unprotected perineum delivery is simple and practical , and it greatly reduces episiotomy rate , perineum injury , maternal pain and the influence on life and thus improves maternal quality of life .So it is worthy of further promotion .
