Effect of different pneumoperitoneum pressure on complications after laparoscopic ovarian cystectomy

Objective To explore the influence of different pneumoperitoneum pressure on the complications after laparoscopic ovarian cystectomy.Methods A prospective selection of 150 patients who underwent laparoscopic ovarian cyst resection in Huludao Central Hospital in recent years was divided into high pressure group(14~15 mmHg)and low pressure group(10~11 mmHg)according to the pneumoperitoneal pressure 75 cases.The hemodynamics,respiratory and circulatory system indexes,ovarian function indexes and postoperative complications were compared between the two groups before and after surgery.Results Cardiac output(CO),mean arterial pressure(MAP),end tidal carbon dioxide partial pressure(P ET CO 2)of the two groups of patients after operation were higher than that of preoperative(P<0.05),while stroke volume(SV)was lower than that of the same group(P<0.05);the postoperative changes of the hemodynamic indexes and P ET CO 2 in the high pressure group were more obvious than the low pressure group,the difference was statistically significant(P<0.05).The postoperative follicular estrogen(FSH)and luteinizing hormone(LH)of the two groups of patients were higher than those of the same group before operation(P<0.05),while estradiol(E2)was lower than that of the same group before operation(P<0.05).The changes in ovarian functional hormone indexes were more obvious in the high pressure group after operation,which was compared with low pressure.Group comparison,the difference was statistically significant(P<0.05).The postoperative complication rate in the high pressure group(62.7%)was significantly higher than that in the low pressure group(25.3%),and the difference was statistically significant(P<0.05).Conclusion Different pneumoperitoneum pressure has different effects on hemodynamics,respiratory and circulatory system and ovarian function in laparoscopic ovarian cystectomy.High pneumoperitoneum pressure can increase the risk of postoperative complications.It is suggested to choose low pneumoperitoneum pressure in combination with the actual situation during operation.
