医院感染管理中的主要矛盾与化解策略

目的通过对当前医院感染管理现状的分析，以改善医院感染管理的现行机制，促进医院感染管理的可持续发展，促使医院感染管理与控制工作落于实处。方法根据2012年9月在广东番禹召开的“全国医院感染管理秋季年会”和“2013年广东省春季医院感染岗位培训班”上各个学员的交流意见进行研究：汇总和总结“上海国际医院感染控制论坛”，检索相关的医院感染管理等一系列的工作，深入的透视，分析，探讨医院感染管理的主要问题和矛盾，并且提出相应的化解策略。结果医院的经济实力和专职人员的素质有限，是影响医院感染管理的主要原因。结论合理收取一些医院感染管理预防成本，实行按不同病种分类付费的制度，加强医务人员全员培训，尤其是应加强各地各医疗单位医务人员培训．增强医务工作者对医院感染防控意识，建立一批有较高素质的医院感染管理团队，建立各项与医院感染管理相关的制度、职责、流程，成立医院感染管理质量控制网，各科成立院感质控小组，制订相关人员职责，做到人人参与院感监控，制订医院感染管理质量奖惩方案，确保将医院感染控制落于实处。

The Principal Contradictions and Countermeasures in Hospital Infection Management

Objective To improve the present mechanism of nosocomial infection management, promote the sustainable develop- ment of nosocomial infection management, and spur nosocomial infection management to be implemented in actual through the analysis of current status of nosocomial infection management. Methods The main problems and contradictions existing in hospital infection management were discussed and related helpful methods were offered through the exhaustive analysis of the trainees＂ ex- changed ideas from ＂Autumn Annual Meeting of the National Hospital Infection Management＂ held in Guangdong Panyu in September, 2012 and ＂Hospital Infection Post Training Class in Guangdong in Spring 2013＂, the summary of the speeches of all members in Shanghai International Forum for Infection Control and Prevention and the retrieval of related literature. Results The limited hospital＇s economic strength and full-time staff＇s quality are the main factors affecting hospital infection management. Conclusion Hospitals charge reasonable cost for hospital infection prevention, establish some payment systems according to different diseases, strengthen the training of every medical personnel on nosocomial infection management especially the medical workers from various medical units and improve their consciousness of hospital infection prevention and control, set up a management team of hospital infection with high quality, establish some management systems, responsibilities and processes that are related to hospital infection, Set up a quality control network of hospital infection management. Every department sets up a team for hospital infection control, formulates relevant personnel responsibility so as to make everyone involve in the monitoring of hospital infection and design the reward and punishment scheme for the quality of hospital infection management, ensure that the control of hospital infection in practice.
