腹腔镜全子宫切除术加阴道顶端悬吊治疗子宫脱垂疗效分析

目的探讨脱垂子宫行腹腔镜全子宫切除手术后，施加阴道顶端悬吊的治疗效果。方法针对Ⅲ度子宫严重脱垂合并阴道膨出的患者30例，采用了腹腔镜全子宫切除并施加阴道残端悬吊治疗。对30例手术效果进行回顾性分析。结果手术时间均处于（117．9±11．9）min，出血量均为（120．5±21．9）mL，术后患者的尿管留置时间为（47±4）h，住院时间均为（6．9±2．5）d，30例患者中发生泌尿系统、肠道损伤等并发症的例数为0，术后随访0．50-1．25年，无一例出现阴道脱垂、张力性尿失禁并发症发生，性生活满意度提高。结论子宫脱垂的患者进行腹腔镜全子宫切除，施加阴道顶端悬吊治疗，手术较为彻底，术后疼痛感低，疗效较为显著，有效防止并发症发生，提高患者的身体素质和生活质量。

Effect analysis of total laparoscopic hysterectomy plus vaginal top suspen- sion in the treatment of uterine prolapse

Objective To investigate the therapeutic effect of total laparoscopic hysterectomy plus vaginal top suspen- sion in the treatment of uterine prolapse. Methods Thirty cases of patients with procidentia combined with vaginal pro- lapse received the treatment of total laparoscopic hysterectomy plus vaginal top suspension. And a retrospective analy- sis of operation effect on 30 cases was conducted. Results All the operation time was in （117.9~11.9） minutes, bleed- ing volume was （120.5＋21.9） mL, postoperative indwelling catheter time was （47＋4） h, the length of stay was （6.9~ 2.5） days. In 30 patients, no case occurred complication of urinary system and gastrointestinal injury. During the post- operative follow-up period for 0.50 to 1.25 years, no case occurred complication of vaginal prolapse or stress inconti- nence urine, and their sexual satisfaction were improved. Conclusion Total laparoscopic hysterectomy plus vaginal suspension in the treatment of uterine prolapse has some advantages, such as more aggressive surgery, less postopera- tive painful, more significant effect, preventing complication, and improving physical quality and life aualitv.
