乳腺肿块穿刺组织学诊断探析

目的探讨超声引导下的空芯针组织学穿刺活检（core neddle biopsy，CNB）在乳腺肿块术前诊断的临床应用价值。方法选取本院收治的198例乳腺肿块患者，采用空芯针穿刺活检获取组织条行石蜡切片病理诊断及免疫组织化学检查。对比CNB与手术切除肿垛（EP）病理活检的诊断符合率。结果198例乳腺疾病患者经CNB诊断为恶性157例，良性41例，CNB与手术切除组织病理学活检的病理诊断符合率为98．9％．CNB病理诊断的良性乳腺肿块符合率为95．1％，恶性肿瘤符合率为100．0％。结论超声引导下乳腺空芯针穿刺活检是一种操作简单、快速、安全并具重复性的术前诊断方法，能比较准确地存手术前确定肿块的病理性质，具有重要的临床应用价值。

Application of core needle biopsy in the diagnosis of breast mass

Objective To evaluate the clinical value of core needle biopsy （CNB） as a preoperative diagnostic modality for breast lesions. Methods One hundred and ninety-eight cases of patients with breast mass were collected, all patients were given core needle biopsy tissue with paraffin pathological diagnosis and immunohistochemical examination. Diagnostic coincidence rate of CNB and EP pathological biopsy were compared. Results Out of the total 198 cases, CNB showed 157 cases with malignant breast masses and 41 cases with benign disease. Diagnostic coincidence rate of CNB and EP pathological biopsy was 98.9%,CNB coincidence rate of malignancy was 100.0% and coincidence rate of benign disease was 95.1%. Conclusion Uhrasound-guided core needle biopsy is minimally invasive, simple, safe and accurate to diagnose breast lesion. It can enable relative accurately confirm the pathological quality of the tumor before clinical operation, and has demonstrated the widespread clinical practice prospect.
