不同挤管方式对脑室引流效果的影响

目的探讨脑室出血微创脑室引流术后引流管的护理方法。方法对部分脑室出血患者微创脑室引流术后引流管的护理,采取先夹闭引流管,然后由近端向远端夹子方向挤压管壁,再松开夹子的疏通引流方法（观察组）;对其他患者的脑室引流管采用常规的挤压引流管方法（对照组）;比较两组引流效果和并发症发生情况。结果与对照组相比,观察组的引流时间明显缩短（P〈0.01）;观察组脑室感染与张力性气脑等并发症发生率低于对照组（P〈0.05,P〈0.01）;观察组引流效果优于对照组（P〈0.01）,病死率低于对照组（P〈0.01）。结论改进的引流管护理方法使引流充分,引流时间缩短,能有效地降低颅内压,减少脑部并发症,降低患者的病死率。

Observation on the effectiveness of the ventricular drainage with different squeeze tube methods

Objective To explore nursing way to the drain of minimally invasive ventricular drainage for the intraventricular hemorrhage.Methods The method of compressing drainage catheter by which the distal end of the draining tube was clipped firstly, then squeezed from the proximal to the distal and disclosed again finally was used in some of the cases who had been administered with the micro-invasive ventricular surgery because of suffering from the intraventricular hemorrhage(improved group).The routine nursing in promoting drain was used in the other patients(control group).Postoperative complications were compared between the two groups as well as the draining effectiveness of two ways to nursing hose.Results The draining time of the improved group was markedly shortened compared to the control group(P<0.01).The puncture-associated complications including rebleeding,intracranial infection after ventricular drainage surgery and tension pneumocephalus were significandy decreased in the improved group compared with the control one(P<0.05,P<0.01).The drainage effect of the experimental group was better than that of the control group(P<0.01 ), and the patient’ s mortality declined(P<0.01).Conclusion The results indicate that this novel approach to nursing drain has some advantages,such as shortening draining time,complete drainage and good effect of reducing intracranial pressure,and reduce cerebral complication and mortality rate.
