幽门螺杆菌感染与脑梗死患者凝血功能及颈动脉粥样硬化斑块稳定性的关系

摘    要:目的探讨幽门螺杆菌(Hp)感染与脑梗死患者凝血功能及颈动脉粥样硬化斑块稳定性的关系。方法选择脑梗死患者共127例,进行颈动脉超声检查、Hp感染的检测、凝血功能(PT、APTT、TT、FIB)及Hs-CRP检测,依据超声检测结果分为不稳定斑块组(85例)和稳定斑块组(42例),依据Hp感染的检测结果分为Hp(-)组(53例)和Hp(+)组(74例),比较各组一般情况和各项检测指标变化。结果不稳定斑块组Hp感染阳性率、Hs-CRP与FIB水平显著高于稳定斑块组,其APTT水平则显著低于稳定斑块组(P0.05)。Hp(+)组斑块不稳定比率、Hs-CRP、FIB水平显著高于Hp(-)组,APTT则显著低于Hp(-)组(P0.05)。结论在脑梗死患者中,Hp感染与颈动脉粥样硬化不稳定斑块和凝血功能异常有密切联系,促进动脉粥样硬化与脑梗死的发生、发展。

Study on relationship between Helicobacter pylori infection and coagulation function in patients with cerebral infarction and its correla- tion to stability of carotid atherosclerotic plaque.

Abstract:Objective To explore the relationship between Helicobacter pylofi infection, coagulation function and stability of carotid ath- erosclerotic plaque in patients with cerebral infarction. Methods A total of 127 patients with cerebral infarction in this hospital were collected. Carotid ultrasound, Hp infection detection, coagulation parameters ( PT, APTT, TT, FIB) and Hs - CRP detection were undertaken. Based on the results of ultrasonic imaging, they were divided into unstable plaque group (85 eases) and stable plaque group (42 cases). They were further divided into Hp ( - ) group (53 cases) and Hp ( + ) group (74 cases) according to the result of detection of Hp infection. General situation and changes in indicators were compared between these groups. Results In unstable plaque group, positive rate of Hp infection and levels of Hs - CRP and FIB were significantly higher than those of stable plaque group, and APTr level was significantly lower than that of stable plaque group ( P 〈 0.05 ). In Hp ( + ) group, the proportion of plaque instability, levels of Hs - CRP and FIB were significamly higher than those of Hp ( - ) group, and level of APTT was significantly lowered ( P 〈 0. 05 ). Conclusion Hp infection, instability of carotid atherosclerotic plaque and coagulation abnormalities are closely linked, thereby Hp infection may promote the occurrence and development of atheroselerosis and cerebral infarction.
