超敏C反应蛋白及血常规对小儿肺炎支原体感染的临床诊断价值分析

摘    要:目的探讨超敏C反应蛋白及血常规对小儿肺炎支原体感染的临床诊断价值。方法分析2011年8月至2013年9月期间收治的140例肺炎支原体感染患儿和150例同期体检健康儿童超敏C反应蛋白及血常规检测结果,同时观察肺炎支原体感染患儿的临床症状、体征缓解情况。结果实验组患儿的超敏C反应蛋白(15.8±2.1)mg/L、外周血白细胞计数(14.8±1.9)×109/L、中性粒细胞比率(71.4±10.4%)、单核细胞比率(5.5±0.9%)均明显高于对照组,淋巴细胞比率(23.7±3.2%)明显低于对照组;超敏C反应蛋白检测敏感度(95.0%)显著高于白细胞计数检测的敏感度(75.7%,χ2=20.831,P0.01);超敏C反应蛋白水平与咳嗽、咳痰、气喘、肺部罗音缓解时间呈正相关关系。结论超敏C反应蛋白水平检测能够较白细胞计数更为准确地诊断小儿肺炎支原体感染,且与临床症状和体征的缓解具有良好相关性,是理想的辅助检查指标。

Abstract:Objective To study the clinical diagnosis value of high-sensitivity C-reactive protein( hs-CRP)and blood routine test in children infected with Mycoplasma pneumoniae( M. pneumoniae). Methods Children infected with M. pneumoniae in our hospital from August 2011 to September 2013 were enrolled in observation group and healthy children received physical examination in the same period were enrolled in control group. The results of hs-CRP and blood routine examinations and clinical manifestations of children infected with M. pneumoniae were observed. Results Compared with control group,hs-CRP level(15. 8 ± 2. 1 ml/L),white blood cell count(14. 8 ± 1. 9 × 109/L),neutro-phil granulocyte ratio(71. 4 ± 10. 4%),and monocyte ratio(5. 5 ± 0. 9%)were significantly higher;while lymphocyte ratio(23. 7 ± 3. 2%) was significantly lower in observation group. The positive rate(95%)of hs-CRP was significantly higher than that of blood routine test. The lev-el of hs-CRP was positively correlated with the remission time of cough,expectoration,asthma and pulmonary rales. Conclusion The detection of hs-CRP may better indicate M. pneumoniae infection than blood routine test and hs-CRP level is positively correlated with the remission of clinical symptoms and signs.
