腹腔镜手术治疗出血性休克型异位妊娠的临床研究

目的探讨腹腔镜手术治疗出血性休克型异位妊娠的临床效果。方法选择妇产科2010年4月至2013年6月收治的异位妊娠伴出血孕妇124例,随机分成实验组和对照组,各62例,对照组采用传统开腹术,实验组采用腹腔镜手术。结果实验组的术中出血量和输血量较对照组明显减少,差异显著(均P0.05)。实验组的术后胃肠功能恢复时间、术后下床活动时间、住院天数、术后镇痛例数均明显少于对照组,差异显著(P0.05)。两组患者皮肤切开愈合情况良好,均为甲级愈合,两组术中术后均无并发症发生。结论抗休克治疗的同时进行腹腔镜手术,术中要求手术者技术熟练,同时做好严密的监测,腹腔镜手术治疗出血性休克型异位妊娠可以顺利进行,得到满意的效果。

Clinical study on treatment of laparoscopic operation for ectopic pregnancy hemorrhagic shock.

Objective To investigate the clinical effect of laparoscopic operation in the treatment of hemorrhage of ectopic pregnancy with hypovolemic shock. Methods Our hospital obstetrics and Gynecology 2010 ectopic from April to 2013 June in our hospital 124 cases of pregnant women with pregnancy bleeding, were randomly divided into experimental group and controlgroup with 62 eases in each group, the control group with open surgery, the experimental group were treated with laparoscopic operation. Results The intraoperative bleeding and transfusion volume decreased significantly, compared with the control group have significant difference, there was statistical significance （ P 〈 0.05 ）. Time, postop- erative recovery of gastrointestinal function after operation in experimental group after time out of bed, hospital stay, postoperative analgesia cases were significantly less than that of the control group, the difference was significant, with statistical significance （ P 〈 0.05 ）. Two groups of pa- tients with skin incisionhealed well, were healed, two patients without postoperative complications. Conclusion The anti shock treatment of lapa- roscopic operation, intraoperative requires skilled operation, and makestriet monitoring, laparoscopic operation in the treatment of ectopic pregnan- cy hemorrhage can be carried out smoothly, obtain satisfactory effect.
