改良Pemberton髋臼截骨治疗大龄儿童发育性髋脱位

目的探讨治疗大龄儿童发育性髋关节脱位（developmental dislocation of thehip，DDH）更为安全可靠的手术方法。方法2000年2月-2006年2月，对DDH髋臼浅而小、股骨头大的19例21个髋关节，在通常术式的基础上，对Pemberton截骨术式进行改良，将截骨点下移，扩大翻转骨瓣面积；取股骨截骨段植骨。同时行股骨转子下短缩外旋截骨，髋人子石膏固定2～3周，3周去除石膏固定改用患肢皮牵引并行仰卧起坐髋关节功能锻炼，8周扶拐行走。术后观察髋关节功能，X线片显示的截骨愈合程度、髋臼对股骨头包容程度及有无股骨头坏死等。结果随访6个月～1年8个月，按照周永德疗效评定标准，总计26～30分14例14髋，21～25分3例4髋，16—20分2例1髋，15分以下1例1髋，总优良率达90％。结论改良Pemberton截骨治疗发育性髋关节脱位可增大股骨头包容，避免了传统术式减少髋臼容积的缺点，更有利于稳定关节，髋关节功能好，股骨头坏死发生率低，是治疗大龄儿童发育性髋脱位较好的手术方法。

Reformative Pemberton acetabular osteotomy for treating older child developmental dislocation of the hip
