胆管-空肠侧侧吻合术的临床试验研究

目的：比较胆肠Roux-en-y侧侧吻合术与端端吻合术两种术式，指导临床医师选择合理的术式。方法：将符合条件的病人随机分成两组。每组70人，用单盲法对两组病人分别流行端端和侧侧吻合术，测量胆道空肠压力差及pH值，并行t检验，同时对术后吻合口漏和狭窄的发生率进行X^2检验。结果：侧侧吻合较端端吻合压力差大（＞5mmHg),pH值变化及两组漏的发生率无统计学意义。吻合口狭窄1，3，5年的发生率组间差异显，P＜0．05，且呈逐年增加的趋势。结论：胆肠侧侧吻合术的长期效果优于端端吻合术，值得临床推广应用。

Experimental study of clinical on bile-duct and jejunum side-to-dide anastomosis

Objective:It is the cmparison study that the two the operation methods Between Roux-en-y of end-to-end anastomosis and side-to-side anstomosis of bile-duct and jejunum.To direct elinical doctor selected operatve procedure.Methods:The patients have divided two groups with conform to study in condition,there was 70 cases each group,single-bilind trial has been used,one group is used to end-to-end anastomosis,the other group side-to side,the measured between bile duct pressure variance and PH bile duct.Moreover,t test has been cheched up,and incidence rate used to chisquare test with fistula and strictures of anastomosis orifice.Results:Side-to-side anastomosis compared with end-to-end,presure variation of between bile duct and jejunum(>5mmHg,in side-to-side group),but pH and incidence rate of fistula of anastomosis orifice were mot statistis significance between the two groups,but there were remarkable difference in strictures of anatomosis on the 1,3,5 year( P <0 05),and more and more from year to year.Conclusion:Longtime effect is better side-to-side anastomosis of bile duct and jejunum than end-to-end anastomosis,htis procedure is highly recommended.
